This application is an add-on to the full version of OKAKI’s Community Health and Immunization Program (CHIP) suite of products. CHIP is a SaaS based system and is not used by the public. It was designed with a simple layout to allow quick, easy, and secure immunization record entry in community settings such as schools, mass immunization clinics, or the homes of vulnerable individuals who cannot be transported to immunization clinics. This application is not for use by the public and only is used by regulated health professionals whose organizations have a current subscription to CHIP.The application requests permission to use the camera so the application can scan barcodes for patient searches as well as vaccine vials for GTIN codes, lot numbers and expiry dates. The application also requests permission to use the microphone to enable the user to use voice to text when entering notes. Use of the camera and microphone is optional as the application is fully functional except for barcode scanning and voice to text. This application does not access any user data on the phone (contacts, photos, mail etc.) and only requires a connection to the internet (Wi-Fi or cellular data). Regulated healthcare professionals that immunize use this app to view a patient’s past immunizations, allergies, and any history of past adverse reactions, and to enter new immunization records. The patient information displayed on the mobile device is retrieved securely by authorized users of CHIP. Access, use and disclosure of personal health information is governed by health information privacy laws and regulations across Canada. No patient information shown in the application is stored on the mobile device.
